Donna’s Diary, an online diary of a fictitious nursing home from the perspective of a director of nursing (DON), is a learning and sharing tool to help nursing homes achieve national health care quality goals and support participation in the National Nursing Home Quality Care Collaborative.
Participating nursing homes work to improve overall quality of care and to reduce hospital readmissions and unnecessary use of antipsychotic medications.
Donna’s Diary was developed by Stratis Health. Stratis Health serves Minnesota as part of Lake Superior Quality Innovation Network (QIN) under contract to the Centers for Medicare & Medicaid Services, in partnership with MPRO (Michigan) and MetaStar (Wisconsin). Lake Superior QIN promotes participation in the National Nursing Home Quality Care Collaborative.
